The kit arrives on a Monday in the kind of box you keep afterwards. It promises to tell you your stress hormone before breakfast, which sounds like exactly the thing a stressed person should know.

On Tuesday your alarm goes at 6:15. You answer two texts, let the dog out and read an email that opens with "Quick question". At 6:40 you remember the saliva strip on the bathroom counter. Of course it is. Cortisol is designed to surge in the first half hour after you wake, so one reading mostly tells you what time you tested. A cortisol test at home pays for its box only when you use it to draw the shape of a whole day.

The short answer: A cortisol test at home is useful only when it maps a whole day. Cortisol surges in the first 30 minutes after waking, so a single morning reading mostly records what time you tested. Take four saliva readings on three ordinary weekdays, and see your family doctor if the late-night reading stays high on three ordinary nights.

6:40 a.m.: why is my morning cortisol always high?

Your body is not panicking about the email. It is getting you vertical. Researchers call this the cortisol awakening response, and pooled German data from 509 adults put the rise at roughly half again in the first 30 minutes.

That makes the morning strip a race against the clock. When an international panel that included McGill's Jens Pruessner wrote guidelines for measuring this rise, their central warning was about timing. Scientists struggle to get it right with volunteers in controlled studies. You are doing it while looking for a dog leash.

The same German data carries a second surprise. On any given morning, about one person in four shows no rise at all. One odd morning is ordinary, so only a pattern across several days means anything.

Noon: what does a cortisol test at home actually read?

The kit most Canadian women will meet first comes from Eli Health in Montreal. Its Hormometer reads cortisol from saliva and sends a result to your phone in about 20 minutes. The company sells it as a wellness device and makes no diagnostic claim, which is refreshingly honest for a category that loves the word "clinical".

Its three-month plan costs US$199 for 24 tests. A useful curve takes four readings, so the plan buys six days of full pictures, which is enough for a proper look and not much more.

Saliva measures free cortisol, the active fraction. A standard blood test measures the total, including the cortisol bound to carrier proteins, and the pill raises those carriers. In a Czech study of young women, three months on a combined pill nearly doubled blood cortisol while saliva barely moved. If you are on the pill, your blood work and your saliva can disagree and both be right.

A patch that reads cortisol continuously, the way glucose sensors do, has been shown by Adaptyx Biosciences. It has not been cleared by the FDA yet, so for now the spit strip is the consumer option.

3 p.m.: cortisol face, cortisol belly and other things the internet diagnosed

By mid-afternoon your feed has an opinion. A puffy jaw means cortisol face, a soft middle means cortisol belly, and tiredness means your adrenals are "burnt out". The cure is usually a tea or a capsule with a leaf on the label.

The Endocrine Society says there is no scientific proof that adrenal fatigue exists as a medical condition. A Brazilian team went further and reviewed the research so thoroughly that they titled their paper "Adrenal fatigue does not exist." The society adds a warning about adrenal hormone pills taken without need: they can make your own glands stop working, and those glands can stay quiet for months after you stop.

The afternoon slump itself is real. It just has more boring causes, such as a short night, a skipped lunch and a third coffee that did nothing.

By lights-out your cortisol should be near the floor. This is the strip that matters most, because a late-night saliva test is one of the first tests endocrinologists use when they suspect Cushing's syndrome, the rare condition the "cortisol face" videos are borrowing from.

A high bedtime reading on one night means you watched something tense or answered a work message in bed. A high reading on three ordinary nights is worth a conversation with your family doctor. Of every number the kit produces, the bedtime one is the only one with a clinical cousin.

What you do in that last hour is the part you control. Put the phone in another room, turn the lamps down, and give yourself a small ritual that signals the day is over.

Should I stop the pill before a blood cortisol test?

Only if your doctor agrees. In a Dutch study of women on oral contraceptives, most had a false-positive result on a standard cortisol suppression test, and results were normal six weeks after stopping. Doctors often time the test to the pill-free week for this reason.

Does ashwagandha lower cortisol, and is it safe?

Some small trials report lower cortisol. In August 2026, Health Canada's safety review concluded that a possible link between oral ashwagandha and liver injury cannot be ruled out. There was no recall, but anyone with liver concerns should ask a pharmacist first.

Do women's cortisol curves differ from men's?

The morning climb is about the same. In the pooled German data, women's cortisol took significantly longer to come back down afterwards, so a woman's morning plateau can last a little longer than her partner's.
