The marker squeaks before anyone speaks. Your friend's oncologist draws a line on the whiteboard and splits it in two. You hold the notebook, because she asked you to write everything down.
Your friend has triple-negative breast cancer, and it has spread. This type is more common in women under 40. It also lacks the three markers most breast cancer drugs aim at, so for decades the first treatment was simply chemotherapy.
In 2026, two new drugs were approved in the U.S. as a first treatment. One of them helped women live about five months longer. Neither is approved in Canada for this use yet, and that gap is what your notebook is for.
| Datroway | For women who can't have immunotherapy. Helped them live longer than chemotherapy did. |
|---|---|
| Trodelvy | Alone, or with the immunotherapy drug Keytruda. Kept the cancer in check longer; too soon to know about survival. |
| In Canada | Both under review by Health Canada as of 31 August 2026. Not yet covered as a first treatment. |
How the new drugs work
Think of chemotherapy sent by courier, with the address written on the box. Each drug is an antibody carrying a dose of chemotherapy. The antibody finds a protein on the outside of the cancer cell, latches on and is pulled inside.
Ordinary chemotherapy travels all over the body. These drugs send most of the dose to the tumour. Side effects still happen, but the hit is more focused.
Which branch is hers?
The first question is whether your friend can have immunotherapy, a treatment that helps the immune system fight the cancer. About seven in ten women with her diagnosis can't, according to the TNBC Foundation. For them, Datroway now has the strongest evidence. On a whiteboard, five extra months is one short line. In a life, it is a summer.
Trodelvy covers both groups. Its survival results aren't in yet, partly because many women in the comparison group switched to Trodelvy once their cancer grew. There is also a small irony in the dates. Trodelvy pioneered this kind of treatment in 2021, yet Datroway reached this year's approval first, by a month.
Can you get them in Canada?
Not through provincial coverage yet. In 2021, Trodelvy took almost two years to go from U.S. approval to a finished price deal with the provinces. This time Canada is running its safety review and its value review side by side, which should be faster.
In the meantime, three routes can work: a clinical trial, a private drug plan, or Health Canada's Special Access Program, which lets a doctor request an unapproved drug for one patient.
Three questions for the next appointment
- "Can she have immunotherapy?" This decides which drug fits her.
- "Is there a trial nearby that uses either drug?" It is often the fastest way in.
- "What does she lose by starting chemotherapy while we wait?" Ask for the answer in months.
Write the answers next to the dates. When Health Canada decides, her doctor will be ready the same week.
Questions readers ask
Why do the two drugs have different side effects?
They aim at the same protein on cancer cells, called TROP2, but carry different chemotherapy. Datroway more often causes mouth sores. Trodelvy more often lowers white blood cells, which raises the risk of infection.
Has Europe approved them?
Partly. The European Union approved Trodelvy as a first treatment in June 2026. A few weeks later, the EU's medicines committee recommended Datroway, which is usually the last step before approval.
How do I find a clinical trial in Canada?
Search ClinicalTrials.gov, a free government website, for "triple negative breast cancer" and set the location to your city. Each trial has a number starting with NCT. Bring it to the oncologist, who can check whether she qualifies.
